OLVG

If you cannot log in to MijnOLVG with DigiD, for example because you do not have a Dutch identity document, you can log in using a username and password. To do this, you first need an activation code.

Step 1: Request an activation code

You can obtain an activation code in several ways:

  • From an outpatient clinic employee.
  • At the the Patient Service Desk (‘Patiënten Servicepunt’) at OLVG, location East or West.
  • By requesting one yourself at mijnolvg.nl. An email address is required.

If you request the code online, click ‘Aanmelden met een activatiecode’ and then ‘Aanmelden met uw informatie’. Enter your details and click 'Verzenden'. If your information matches an existing patient record, you will receive an activation code by email.

Step 2: Create your account

Use the activation code to create your account.

  • Choose a username (maximum 20 characters)
  • Create a password. Your password must:
    -   contain at least 8 characters;
    -   include at least one letter and one number;
    -   be different from your username.

Step 3: Complete the login process

You will receive a verification code by email or text message. Enter this code to activate your account and complete the login process.

Did not receive an activation code?

In some cases, we may not be able to send you an activation code. This can happen if:

  • your details do not match an existing patient record;
  • you already have a MijnOLVG account.

Already have an account?

You can use the MijnOLVG website or app to recover your username or reset your password.

Not yet a patient at OLVG?

Please contact OLVG to register as a patient. Once you are registered, you can request an activation code.
